When leaving the university, you must submit a letter of resignation to your manager and HR providing your last day of work. For retirements, you may want to first consult with SURS to determine the timing of your retirement. You must also complete an exit checklist and submit it to HR prior to your last day of employment.

  • Complete the Transmittal Form
  • Submit the form and a letter providing rationale for the status to the Director of SLCL
  • The packet is submitted to LAS for review and approval
  • The Provost will provide a letter to the faculty member to provide an update on the request
